Office Forms
Attendance Note
Attendance excuses are to be submitted through Skyward. We would prefer you submit absences through Skyward, but if you are not able to, please use this form. This is what we call the "green note" in the office. Use this note to report an absence or tardy in the event that you did not put it in Skyward or to inform the office of an early dismissal.
Transportation Note
This is our "yellow note". Use this note for any change in transportation or if a child will be a parent pick up.
Educational Trip
Click this to fill out an educational trip request form. Please be sure to read the details on this form to understand the CV educational field trip policy. This form is to be used for trips lasting less than 10 days. If you are taking a trip lasting 10 days or longer please contact the office.

Physical Form
Click on this link to go to the Physician's Report Of Physical Examinations. Pennsylvania School Health Law requires all children to have a medical examination upon original entry into school, in sixth grade and in eleventh grade.
Dental Form
